i'm need desperate help!!!

i'm searching for someone to make me a very small and basic site:

 

the user comes to the page(with no background and stuff) and enters:

name

location

e-mail

eventually phone

 

all this is put on a list where i can see how many goa-heads there are in romania,where they are from and then send them an e-mail.

 

 

please help me!!!!!!!!!!!or at least tell me how i can do it myself

 

 

u will have u're own memorial for helping to build up the romanian goa scene!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!
